How Hope Alzheimer’s and Dementia Care Is Working to Reduce the Financial Burden for Families


Across California, families caring for loved ones with dementia are searching for support that allows their loved one to remain safe, engaged, and connected while continuing to live in the community. Adult day programs are one of the most effective and compassionate models of care available. Yet for many families, cost remains the greatest barrier to access.

At Hope Alzheimer’s and Dementia Care, we believe that no family should have to choose between financial stability and quality dementia care. That belief drives our ongoing commitment to stay informed, proactive, and deeply involved in exploring every available funding pathway, coverage option, and emerging program that may help lower the cost of adult day services for the families we serve.


Why Adult Day Programs Matter in Dementia Care


Adult day programs provide far more than supervision. High-quality programs offer structured routines, cognitive stimulation, social connection, therapeutic activities, emotional support, nutrient-rich foods, and meaningful purpose. For individuals living with dementia, this type of engagement can reduce anxiety, support brain health, and improve overall quality of life.


For caregivers, adult day programs provide essential respite. They allow family members to continue working, attend to their own health, and sustain their caregiving role for the long term. In many cases, adult day care prevents crisis-driven hospitalizations and delays or avoids the need for long-term residential placement.


The Medicare Gap Families Continue to Face


Medicare and Adult Day Programs are something we wish to see at the forefront of our state-wide concerns. Original Medicare does not typically cover adult day care services. Medicare was designed to focus on acute medical care, such as hospital stays, physician visits, and short-term skilled services. Adult day programs, despite their proven health and cost-saving benefits, are still largely categorized as non-medical or custodial care.


Medicare may cover limited social or behavioral health services if they are prescribed by a physician and directly tied to a medical treatment plan. However, adult day programs are generally excluded unless skilled nursing care is being delivered at the same time.

This gap leaves families paying out of pocket for services that clearly support health, dignity, and community living.


Medicare Advantage, PACE, and Limited Pathways


Some Medicare Advantage plans in California offer supplemental benefits that may help with adult day services, transportation, or caregiver support. These benefits vary widely by plan, provider, and county. Coverage is often limited and can change annually.

PACE programs, which combine medical care and adult day health services into one coordinated model, can be an excellent option for eligible individuals. However, PACE is not available in all regions, and eligibility requirements exclude many families who still need adult day support.

One of the most promising developments we are closely following and actively exploring is the Medicare GUIDE Program.


Understanding the Medicare GUIDE Program


GUIDE stands for Guiding an Improved Dementia Experience. It is a new Medicare model designed specifically to support individuals living with dementia and the family caregivers who support them.


The GUIDE Program focuses on comprehensive, coordinated dementia care rather than fragmented services. Its goals include improving the quality of life for individuals with dementia, reducing caregiver stress, preventing avoidable hospitalizations, and keeping people supported in their homes and communities for as long as possible.


Key components of the GUIDE Program include:


• A dedicated care navigator who helps families understand and coordinate services

• Ongoing dementia care management led by an interdisciplinary team

• Education and training for caregivers

• Crisis support and care planning

• Access to respite services for caregivers.


While the GUIDE Program is still being rolled out and refined, it represents a meaningful shift in how Medicare approaches dementia care. It acknowledges that dementia is not just a medical diagnosis but a condition that impacts entire families and requires coordinated community-based support.


Why the GUIDE Program Matters for Adult Day Care


Although the GUIDE Program does not automatically guarantee coverage for adult day programs, it opens the door for new models of care coordination and reimbursement that recognize the value of structured daytime support, caregiver respite, and community engagement.


At Hope, we are now actively evaluating how our adult day program may align with GUIDE care models and how participation in or collaboration with GUIDE providers could help families access broader support and potentially lower out-of-pocket costs over time.


This work requires careful research, collaboration, and advocacy. It also requires staying informed as Medicare continues to test and expand innovative dementia care models.

The need for adult day programs is not going away. Dementia rates are rising, families are stretched thin, and caregivers need real, sustainable support.

The following resources are provided to help families learn more about Medicare, community-based programs, and emerging dementia care models. Hope Alzheimer’s and Dementia Care continues to actively monitor these programs to support families in finding affordable, high-quality care.


Medicare Coverage Information


Original Medicare (Parts A and B) Provides coverage for hospital care, physician services, outpatient care, and limited skilled services. Adult day care is not typically covered unless services are part of a skilled medical treatment plan.


Medicare Advantage Plans (Part C) Offered through private insurers approved by Medicare. Some plans may include supplemental benefits such as transportation, caregiver support, or limited adult day services. Coverage varies by plan and county.


Families are encouraged to review plan documents carefully and contact their insurance provider directly to confirm benefits.


Program of All-Inclusive Care for the Elderly (PACE)


PACE is a comprehensive care program that integrates medical care, long-term services, and adult day health services for eligible older adults.


General eligibility requirements include:


• Age 55 or older

• Living in a PACE service area

• Needing a nursing facility level of care

• Ability to live safely in the community with support


Availability varies by region, and enrollment requirements apply.


Medicare GUIDE Program


Guiding an Improved Dementia Experience


The GUIDE Program is a new Medicare initiative designed to improve care for individuals living with dementia and their caregivers.


Key elements of the GUIDE Program include:


• Coordinated dementia care led by an interdisciplinary team

• Dedicated care navigation for families

• Education and training for caregivers

• Access to caregiver respite services

California Specific Support Resources


Area Agencies on Aging (AAA) Local agencies that provide information, referrals, and support services for older adults and caregivers throughout California.


Medi Cal and Home and Community-Based Services Some families may qualify for state-funded programs that help support care in the community, depending on income and eligibility.


Caregiver Support Programs California offers caregiver education, respite resources, and support groups through local and state-funded programs.


The Alzheimer's Association


The Alzheimer’s Association is one of the largest nonprofit organizations in the world dedicated to Alzheimer’s care, support, research, and advocacy. In addition to a national 24/7 helpline and extensive educational resources, many local chapters offer financial assistance through caregiver respite grant programs.


For example, the Northern California and Northern Nevada Chapter provides a one‑time caregiver respite grant of approximately $1,000 per family to help cover the cost of in‑home care, adult day care, or short‑term facility stays for those living with Alzheimer’s or related dementias. Eligibility typically includes living in the chapter’s service area, providing substantial care, and demonstrating financial need, and the grant funds are paid directly to the service provider after care is provided rather than to the family caregiver.
